Other Important Information:
The Special Needs Alliance and the City of Lubbock Health Department have established the Special Needs Registry of Lubbock County to better serve the special needs of the community in the event of a public health threat. The Special Needs Registry is a free, voluntary program that was created to provide assistance to people with special health needs during times of emergency such as during a natural disaster or terrorist incident.
The information collected is important to first responder efforts and assists the Fire and Police Department during search and rescue operations. It may also be used in the event of a bio-terrorism attack when people may require special medications.
Lubbock County residents may register by printing and completing the Special Needs Registry Form found online at
www.specialneedslubbock.org then mailing or faxing it to the Health Department. Family members or caretakers should encourage their loved ones living with special needs to register. Forms may also be obtained from the following locations:
Public libraries: Groves (5520 19th St.), Mahon (1306 9th St.), and Patterson (1836 Parkway Dr.)
Senior Centers: Lubbock (2001 19th St.), Mae Simmons (2302 Oak Ave.), Rawlings (213 40th St.), and Trejo Supercenter (3200 Amherst St.)
Downtown Citibus station (801 Broadway Ave.)

The information collected is confidential and will only be shared with emergency response agencies to improve their ability to serve. For more information, contact the City of Lubbock Health Department at (806) 775-2953.
